Applicant must fill in necessary application forms, and make ready attached
documents. In principle, applicant should submit these documents
at a service
counter of regional immigration office.
If the applicant is younger
than 16 years old or is unable to submit the application form
because of illness or other reasons, his/her relative may make
application
on behalf of the applicant.

(The photos must be 5cm×5cm, must be taken within 2 months
before the application procedure and cover upper body with uncovered
head, indicating the name and the birth date on the reverse
side.)

- Passport or
Certificate of Status of Residence (if the applicant foreign
national is unable to present passport or Certificate of
Status of Residence,
the foreign national must submit a copy of document that
describes the reason for not presenting it.)
- Alien registration card
(if the applicant has it)
- Applicable certificate if the applicant
foreign national has Permission for Provisional Landing,
Landing permission for Crewmen, Permission for Emergency
Landing, Landing
Permission Due to Disaster, or Landing Permission for Temporary
Refuge; or
- Permit of Provisional Release if the
applicant foreign national is released provisionally.
