Foreign national may file this application if their alien registration certificate
has been damaged or spoiled. Some applicant may not register a new fact
because the
corresponding column is entirely full on his/her alien registration certificate.
In this case, the applicant may receive a new certificate by applying for
changing one of the columns: "Name
in full" of Nationality; or for rectifying the columns: "Name in full," "Date of birth;" , "Sex" or "Nationality."
In this case, applicant needs to
submit Application for Issuance of Registration
Certificate, his/her passport, alien registration certificate and two
photos complying with certain requirements (these photos are not required
for applicants
younger than age 16. |

Foreign nationals may file this application if their alien registration certificate
has been lost due to loss, theft or destruction. Within 14 days after knowing
the fact of having lost the certificate of alien registration, you must
apply for reissuing the certificate. In addition, if you have left Japan
with the re-entry permit, have entered Japan again, but don't have the
certificate, or if you do not have your certificate at the time of receiving
the refugee travel document because you have lost your certificate for
any reason other than being lost due to loss, theft or destruction, you
may apply for reissuing the certificate to the mayor of your domicile within
14 days since reentering Japan.
In this case, applicant needs to
submit certificate application form, his/her passport, two photos complying
with certain requirements (these photos are not required for applicants
younger than age 16.) and any other document that the mayor deems necessary
(such
as a statement of reason, or a certificate proving the said fact). |

Foreign national may file this application if they change their address, extend
your period of stay or change any other registered fact. Within 14 days
after knowing the fact of having changed your address in Japan, name,
nationality, occupation, status of residence, period of stay, or name
and location of
place where the alien is employed or his office , you must apply for
changing the said registered fact to the mayor of your domicile (or the
mayor of
your new domicile, if you have moved to any other municipality). If you
change your address/domicile in your home nation, passport number, the
date of issuing your passport, the name of household er, the personal
relationship to householder, or any other family affairs, you may apply
for changing
the said registered fact to the mayor of your domicile before you file
some applications after the said registered fact occurs.
If you change the registered
address, you need to submit application form for changing the registered
matter, and your alien registration certificate.
If you change the registered other
than your address in Japan, you need to submit application form for changing
the registered matter, and your alien registration certificate as well
as a document that proves your registered fact has changed. |

Foreign nationals (except for those younger than 16 years old) must apply to
the mayor of your domicile for confirming that the registered matters
on the alien registration card represent the actual fact. In this case,
foreign
nationals must in principle file an application within 30 days after
the fifth birthday (or the seventh birthday in the case of permanent
resident
or special permanent resident) since either of the date of new alien
registration or the date of the last time the major confirmed the registered
facts.
If a foreign national is younger
than 16 years old, the foreign national does not need to apply for the
said confirmation until he/she becomes 16 years old. If a foreign national
turns into 16 years old, the foreign national must apply form the said
confirmation within 30 days after the foreign national turns into 16
years old.
In this case, applicant needs to submit
application for confirmation of matters, his/her passport, two photos complying
with certain requirements, and your alien registration certificate. |
